FINANCIAL POLICY
Payment Options: For your convenience, our office accepts personal checks, cash, debit cards, MasterCard, VISA, American Express and Discover.
A payment plan that you can apply for is available through CareCredit.com.
INSURANCE
When making your appointment you will need to provide the following information to our office:
Insurance name & address
Subscriber: Name, Employer, Birthdate, ID# or SS# & Group number
Our office will then contact your insurance company for benefits to determine what will be an estimated copayment.
We will then contact you to let you know what your required estimated copayment will be at the time of service.
The information provided by the insurance company is based on your benefit records on the day we have contacted them and is subject to change.
Your copay is subject to change depending on any outstanding claims not yet in the system when your treatment is billed.
APPOINTMENTS
Out of necessity, treatment is often begun at the first visit.
Dependent upon the case presented to us, treatment may take two or more appointments to complete.
We will make every effort to take you at your appointed time. However, when necessary, we take patients in pain on an emergency basis.
Our goal is to give you the same care and attention that we give all our patients. So, sometimes we could be running behind schedule upon occasion.
If this does happen please accept our apology.
